Advanced Practice Provider (NP or PA) Inpatient New

NJ Palliative Care, in partnership with RWJ Hospital, is seeking a compassionate and skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our inpatient palliative care team. This role focuses on improving quality of life for patients with serious illness through symptom management, goal-directed care, and family support.

Responsibilities:

• Perform comprehensive palliative care assessments for hospitalized patients
• Develop and implement individualized care plans in collaboration with physicians, nursing staff, and other specialists
• Provide expert symptom management, including pain, nausea, dyspnea, and anxiety
• Facilitate discussions with patients and families about goals of care and treatment options
• Participate in daily interdisciplinary rounds and care conferences
• Coordinate care transitions to appropriate post-acute or community-based services
• Maintain accurate documentation in the electronic medical record
• Contribute to quality improvement initiatives and adherence to best practice standards

Qualifications:

• Current NJ license as NP or PA
• Board certification in respective discipline
• Experience in palliative care, hospital medicine, or acute care preferred
