Title: New Grad Nurse Practitioner - Harlem $155k

Location: New York, NY (Onsite)

Employment Type: Full Time, 40 hours/week, 5 days a week

Compensation

Pay Range: $108,500-$155,000 Per Year

Description

Provide medical care in a Skilled Nursing Facility setting.

Conduct regular patient assessments and manage chronic and acute conditions.

Ensure accurate documentation of patient information.

Collaborate with interdisciplinary staff to develop care plans.

Communicate effectively regarding changes in resident conditions.

Key Responsibilities

Perform in-person clinical visits regularly and as needed.

Review and document patient medical histories.

Conduct clinical assessments considering geriatric comorbidities.

Monitor and respond to acute changes in condition promptly.

Ensure accurate documentation of clinical encounters for billing and compliance.

Qualifications

Current and valid license and registration as a Registered Professional Nurse and Nurse Practitioner.

3 years of clinical nursing practice in home care, skilled nursing facility, or hospital.

3500 hours as a Nurse Practitioner with the ability to bill Medicaid and Medicare.

Experience with electronic documentation platforms.
