Position Description

Position Overview:
We are currently seeking licensed Family Nurse Practitioners to join our Team

What You’ll Do:

Perform virtual assessments for patients
Complete all forms and documentation with same-day turnaround
Collaborate with care coordinators, intake teams, and agency staff
Maintain accurate and timely clinical notes

What We’re Looking For:

Active NP or FNP Medicaid Enrolled in NYS
Background in home care, primary care, or geriatrics is a plus
Excellent communication and time management skills
Comfortable with basic tech and virtual care platforms
