SUMMARY OF POSITION:
Penn State Health has an opening for a Nurse Practitioner for Post Acute Care. As part of a multidisciplinary team, patient care will be provided at Long-term care facilities and Rehab Facilities, in conjunction with a Physician Board certified in both Internal Medicine and Geriatric Medicine. This position will include Internal Medicine consultation services for patients in Long Term Care and Rehab Facilities. The NP functions as a member of a multidisciplinary healthcare team and provides a wide range of patient services to a specific patient population across the continuum of healthcare. This position is Monday-Friday, with No weekends, and call rotation that is shared with other providers.

MINIMUM QUALIFICATION(S):
- Masters Degree in Nursing with clinical specialization required.
- Two (2) years' work experience as a RN required.
- Current licensed to practice as a CRNP by Pennsylvania Board of Nurse Examiners required.
- BLS Certification required
- Previous Internal Medicine Experience. Inpatient and/or LTAC-Rehab clinical experience. Experience working with the Geriatric population
